At Changing Lives Network, we specialize in high-impact interventions that foster recovery and independence. Guided by our Comprehensive Clinical Assessment (CCA), we provide tailored Support Services, utilizing specialists with lived experience to offer guidance self-advocacy training. Our Transitional Youth Services specifically support young adults aged 16-21 as they navigate the shift from state custody or school-based services to independent adulthood, focusing on housing, education, and employment to build a sustainable support network. Additionally, we offer a range of outpatient therapy services designed to meet the needs of and families facing mental illnesses, substance abuse issues, and trauma, all within a safe and nurturing environment that promotes healing and positive change.Our Support Specialists are here to help you identify and connect with the resources need to achieve your goals independently. Counselors can refer Medicaid consumers interested in supported self-directed recovery to our peer support services.
